The new Heart & Stroke Hypertension Management Program consists of health care provider education and tools as well as patient specific tools to facilitate blood pressure management and control by providers and self-management by patients. More than 3,600 patients participated in the Canadian study. Remarkably after three years, patients with hypertension sustained average blood pressure reductions of 6.4/3.8 mmHg. The Program has resulted in 41 percent more people in 11 primary-care centers across Ontario keeping their hypertension under control.
